We are actively recruiting for an IDT Coordinator to work with our PACE team.
This position requires a dynamic individual, who enjoys working in an environment that encourages developing relationships with our participants and their caregivers, where a team focused approach to care is the cornerstone of our organization.
SpecificationsEducation:
High School Diploma or GED equivalent required; Related Associates Degree preferred
Experience:
Minimum two (2) years of related clerical or secretarial experience required; PACE experience preferreD.

Valid MI driver's license preferred.
Special Training:
Only acts within scope of practice.
Meet a standardized set of orientation and/or individual competencies for the specific position description established by PACE of Southwest Michigan, and approved by CMS before working independently.
Immunizations:
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Specific Requirements and Considerations:
May be subject to exposure and handling of infectious waste, diseases, conditions or hazardous chemicals, etc.

Ages of Patients Rendered Care:
Adult, GeriatricKey Responsibilities o Types letters, correspondence, forms, reports, meeting minutes and other documents which are often confidential in nature o Attends and serves as administrative support during IDT documenting minutes o Generates letters and documents in participants' medical records o Coordinates care conferences, maintains agendas for morning huddles and IDTs, documents meeting minutes and manages meeting follow-ups o Coordinates process for denial and reduction of service letters o Schedules IDT assessment for participant's reassessment and annual assessments o Coordinates Medicaid redetermination process and works with DHHS Representative and Center Manager to ensure completion and compliance o Interacts with all levels of staff, participants, families and the general public on the phone and in person, offering information and assistance or directing the person to the appropriate department o Attends staff and committee meetings as directed o Serves as office receptionist (back-up) as directed o Assists in the SDR process o Abides by all established PACE of Southwest Michigan policies, rules and regulations o All other duties as assignedPM20Requirements:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
